Requirements:

• Line up early at the PAG-IBIG Branch near you.
• Prepare your PAG-IBIG MID No.
• Fillout PAG-IBIG form (HQP-PFF-108).
• Photocopy of 1 valid govt ID.
• Pay P125 pesos.
• Claim your PAG-IBIG Loyalty Card on the day itself.
